Joint wear and tear – causes, symptoms and treatment

Joint wear and tear, known in medicine as osteoarthritis (or osteoarthritis), is one of the most common degenerative diseases, which mainly affects people of mature and advanced age (over 50 years), but is increasingly found in younger, active individuals. According to data from the World Health Organization (WHO) and global epidemiological studies, the disease affects nearly 701% of the population over the age of 65. Worldwide, statistics show a serious gender difference - about 365 million women and nearly 230 million men suffer from osteoarthritis.

To understand the essence of the problem, it is important to look at what processes occur in the joint itself.

  • Destruction of cartilage tissue – the basis of the disease is the progressive thinning and loss of elasticity of the articular cartilage, namely the smooth tissue that covers the bone surfaces and acts as a natural shock absorber.
  • When cartilage becomes damaged and its surface becomes uneven or disappears completely, the protective cushion is lost and the exposed bone ends begin to rub directly against each other during movement.
  • In response to the unusual friction and stress, the bone tissue beneath the cartilage begins to thicken and the body responds by forming additional bone growths known as osteophytes or "spikes.".
  • The process affects the entire joint, including its inner lining (synovial membrane), which secretes lubricating or synovial fluid. It becomes inflamed by microscopic pieces of detached cartilage, which changes the composition of the joint lubricant and impairs tissue nutrition.

Why are women more affected by joint wear and tear?

The main reason for the higher incidence of osteoarthritis in women lies in the profound hormonal changes that occur with age. Estrogen plays a key protective role for the musculoskeletal system, as it maintains the strength of cartilage tissue and suppresses inflammatory processes in the joints. With the onset of menopause, the levels of this hormone drop sharply, which deprives cartilage of its natural shield and accelerates degenerative processes.

The anatomical features of the female body also have a serious impact on premature wear. The wider pelvis in women changes the angle of load on the knee joints, which creates specific mechanical pressure and leads to their faster damage compared to men.

An additional factor is the innate greater elasticity of ligaments and tendons, due to the biological need for flexibility at birth. However, this increased flexibility and laxity of the ligaments makes the joints more unstable under daily load, which increases microtrauma to the cartilage over the years.

Risk factors for worn joints

Although age is a major factor, degeneration of articular cartilage is rarely due to a single circumstance. The process is usually accelerated by a combination of genetic predisposition, mechanical overload, and certain lifestyle habits.

  • Overweight and obesity - excess weight puts constant mechanical pressure on the weight-bearing joints (knees, hip joints, and spine), while at the same time, adipose tissue produces proteins that maintain chronic inflammation in the body.
  • Injuries and repetitive microtraumas – old fractures, sprains or torn ligaments (especially in athletes) disrupt the biomechanics of the joint and lead to premature local wear.
  • Occupational overload – occupations involving heavy physical labor, prolonged walking, or regular weight lifting subject the cartilage to constant excessive strain.
  • Hereditary defects in the structure of collagen or congenital abnormalities in the shape and alignment of bones can lead to more rapid joint damage at an earlier age.
  • A sedentary lifestyle – lack of movement weakens the muscles surrounding the joint, which should stabilize it, and worsens the circulation of synovial fluid, depriving the cartilage of nutrients.
  • Conditions such as diabetes mellitus or gout alter the body's metabolism and can directly accelerate the destruction of cartilage tissue.

First signs of worn cartilage

In the early stages of osteoarthritis, the body sends subtle signals that people often mistake for simple muscle fatigue, a slight strain, or a change in weather. For this reason, the first warning signs are widely ignored in the hope that the discomfort will go away on its own after a short rest.

The most characteristic early symptom is the initial pain and stiffness that occurs in the morning immediately after waking up or after prolonged sitting in one place. These sensations are caused by the temporary stagnation and thickening of synovial fluid in the joint during the night, but usually pass within ten to fifteen minutes after a short movement.

Another early sign is a dull, deep pain that only gets worse with more strenuous activity, such as climbing stairs or walking for a long time, and goes away completely with rest. It is caused by the initial thinning of the cartilage cushion, which causes the underlying bone tissue to bear more mechanical pressure than usual.

At this stage, you may also notice a slight popping, crunching, or a feeling of friction during certain movements. This sound phenomenon is due to the appearance of the first microscopic roughnesses on the previously perfectly smooth surface of the damaged articular cartilage.

Advanced osteoarthritis

Advanced osteoarthritis can affect virtually any joint in the human body, but it most often affects the weight-bearing joints – the knees and hip joints, as well as the spine and small joints of the hands. 

This is because the knees and hips bear the enormous pressure of our own body weight every day, while the spine and fingers are subjected to constant, repetitive mechanical stress from the thousands of movements in everyday life. When the protective cartilage in these areas is almost completely destroyed, the disease enters its advanced phase and the symptoms become permanent.

Worn out hip joint – symptoms

In advanced coxarthrosis (wear and tear of the hip joint), the pain is no longer limited to the hip area, but often spreads to the groin, buttocks, and even down to the knee. 

Because the cartilage is almost gone, the bone surfaces are painfully pressed together with each step, forcing the body to compensate and leading to a characteristic limp. 

Over time, the joint capsule shrinks and large spikes form, which severely limits movement - it becomes extremely difficult and painful for the patient to put on shoes, bend over, or get into a car.

Worn knee joint – symptoms

Advanced wear and tear here (gonarthrosis) is recognized by the constant, cutting pain that does not subside even in a state of complete rest or during sleep. 

Due to the strong friction and mechanical irritation from the remaining cartilage, the inner lining of the joint becomes chronically inflamed, leading to regular swelling and fluid collection (the so-called "water in the knee"). 

At this stage, the damage also causes visible deformity – the legs may curve outward or inward (O-shaped or X-shaped), and the joint itself begins to "lock" or appear unstable due to weakening of the ligaments. 

Worn out joints of the spine

When the process involves the facet joints and intervertebral discs of the spine (spondyloarthrosis), the main symptom is constant, dull pain in the lower back or neck, accompanied by severe stiffness. 

The loss of cartilage and bone spurs begin to physically compress the nearby nerve roots. This mechanical compression causes numbness, tingling, burning sensations, and sharp shooting pain that travels down the length of the leg (sciatica) or into the shoulders and arms. 

Wear and tear on the small joints of the hands

In advanced stages, osteoarthritis of the hands leads to irreversible structural changes that primarily affect the distal phalanges of the fingers and the base of the thumb. Chronic inflammation and the bone's attempts to repair itself by accumulating new tissue lead to the appearance of hard, painful nodes (Heberden's and Bouchard's nodes). The fingers become visibly curved and thickened, and fine motor skills deteriorate. Due to pain and loss of grip strength, everyday activities such as turning a key, opening a jar, or buttoning become a serious challenge.

Worn out joints – treatment

Dealing with osteoarthritis requires a comprehensive approach, as once damaged, joint cartilage cannot repair itself. It is important to understand that lasting success and relief are only achieved through long-term and comprehensive lifestyle changes, not one-off or temporary measures.

What are the main management and treatment options?

  1. Dietary changes and weight control – losing excess weight is the most effective way to directly reduce mechanical stress on the knees and hip joints. Including foods with anti-inflammatory effects helps to control internal destruction processes.
  1. Exercise therapy and physiotherapy (kinesitherapy). Specially selected low-impact exercises (swimming, cycling, water aerobics) strengthen the muscles around the affected joints. Strong muscles take on some of the weight and stabilize the joint, which protects the cartilage from further microtrauma.
  1. Taking specialized supplements supports cartilage metabolism. They improve the quality of synovial fluid and slow down degenerative processes.
  1. Drug therapy with nonsteroidal anti-inflammatory drugs (NSAIDs) and analgesics in the form of tablets or gels for topical application. Their aim is to temporarily suppress pain and control acute phases of inflammation to allow the patient to remain mobile.
  1. For more serious complaints, direct injections into the joint (most often with hyaluronic acid or corticosteroids) are administered. These act as a „liquid prosthesis“ that lubricates the joint from the inside and provides rapid, albeit temporary, relief of symptoms.
  1. When the cartilage is completely gone and conservative methods are ineffective, surgery is resorted to. Most often, this involves replacing the damaged joint with an artificial one (endoprosthesis), which completely restores mobility and eliminates pain.

Nutritional supplements for worn joints

Nutritional supplements are an excellent aid in the fight against osteoarthritis, as they provide the body with concentrated building blocks that are difficult to obtain in the necessary quantities through the daily menu.
