Benfotiamine is a lipid-soluble derivative of thiamine. The increased lipid solubility of benfotiamine allows it to more readily penetrate nerve cells. After oral administration, benfotiamine exhibits increased bioavailability compared to an equivalent dose of water-soluble thiamine. Therefore, benfotiamine may be a better option for the treatment of thiamine deficiency than its water-soluble counterpart.

Thiamine, also known as vitamin B1, plays a vital role in energy metabolism and the breakdown of sugars and carbon skeletons; it is also involved in neuronal communication and immune system activation. Thiamine's association with these vital functions makes it an important dietary nutrient. Reduced thiamine intake leads to impaired energy metabolism, increased cellular oxidative stress, and neurological side effects.

Benfotiamine acts by modulating advanced glycation end products (AGEs). It may also act through AGE-independent pathways.
